Population health planning

COORDINARE, as the South Eastern NSW PHN, bases our population health planning on a broad range of data inputs: national priorities, locally assessed needs, service mapping and utilisation, stakeholder engagement, clinical expertise and consumer feedback.

PHNs have a set of national headline performance indicators to measure their progress on health outcomes. The national headline performance indicators are currently:

COORDINARE’s population health analysis also involves a consultative approach. Consultation and priority-setting will involve our Clinical Councils, Community Advisory Committee, Illawarra Shoalhaven Local Health District, Southern NSW Local Health District, as well as ACT Health and the Capital Health Network in recognition of the considerable flow of patients between the ACT and NSW.
