In the South Eastern NSW region, almost one in five residents is aged 65 years or older, so demand for palliative and end of life care services is increasing.
General practitioners (GPs) develop ongoing and trusted relationships with their patients and are well positioned to initiate and promote advance care planning (ACP) within the general practice setting.
COORDINARE is inviting Expressions of Interest (EOIs) from accredited general practices within South Eastern NSW to embed advance care planning into their practice using The Advance Project™ resources.
The Advance Project™ is a free, evidence-based toolkit and training package designed to support GPs, nurses and practice managers to initiate advance care planning conversations and assess patients’ and carers’ palliative and supportive care needs.
Practices are eligible for up to $25,000 (ex GST) to support the implementation of this project. Applications close Tuesday 29 September 2020.
