Supporting end of life care – Innovation Projects

In NSW, the number of people aged over 65 years is projected to increase by 65% between 2002 and 2021. In the South Eastern NSW region, one in five residents is aged 65 years or older. As a result of our ageing population, demand for palliative and end of life care services is growing in South Eastern NSW. COORDINARE, as the South Eastern NSW Primary Health Network (PHN) aims to promote timely and family-centred palliative and supportive end of life care for all people living in the region.

COORDINARE is inviting Requests For Proposals (RFP) from organisations to facilitate change and innovation in palliative and end of life care. Through this funding, COORDINARE seeks to promote a public health approach to palliative and end of life care to complement the range of existing end of life initiatives currently funded by COORDINARE.
