Strengthening Capacity and Coordination of Community Based Palliative and End of Life in South Eastern NSW - Phase 2

COORDINARE - South Eastern NSW PHN invites expressions of interest (EOI) from suitable organisations or collaborations to apply for funding as part of phase 2 of this initiative to assist in strengthening integrated palliative and end of life care service capacity in the community.

Through this funding, COORDINARE seeks to support improved capacity, coordination and partnerships between providers of palliative and end-of-life care within the South Eastern NSW region. Ultimately, the funding will support improvements to and delivery of high quality palliative and end of life care in the community and an optimal end-of-life experience for individuals, their families and carers.

Organisations involved in community palliative and end of life and end-of-life care delivery within the South Eastern NSW region. This includes:

  • general practices and other primary care providers
  • community pharmacies
  • community health providers (non-government funded)
  • aged care providers

Submissions from consortia and partnerships are strongly encouraged.

A total pool of funding up to $600,000 ex GST will be dispersed through this initiative, with individual funded projects to be a minimum of $35,000 ex GST.

Funded projects will run for 12 months from date of execution. All projects will be subject to an evaluation.

SUBMISSIONS: EOIs must be lodged electronically via the Tenderlink portal. Submissions must be received by 5:00pm AEST on 14 January 2019. Late submissions will not be accepted.
