Quality improvement

COORDINARE is committed to fostering quality in general practice and our quality activities are designed to guide and support general practice to improve patient management through building good systems management.

Practices can choose from quality initiatives that range from practical self-assessment checklists through to whole-of-practice review.

COORDINARE’s QI tools and resources support practices to reflect on the way care is organised and delivered now as well as to prepare for changes in primary care such as the Patient Centred Medical Home.

Support for practice quality improvement targets five domains:

  • building capacity within primary care
  • quality and safety – improving efficiency and effectiveness
  • meaningful use of data to drive care at the practice level
  • digital health and use of technology
  • clinical networks and workforce development

The QI tools and resources will assist your practice to:

  • improve practice workflow processes
  • use technology and patient data more effectively
  • make improvements in the delivery of care
  • improve patient participation in their care
