COORDINARE and our Local Health District (LHD) partners have been working collaboratively to enable all people living across the region to access health services that are provided in a way that responds to their life circumstances; are coordinated across the continuum of care, and are safe, high quality, accessible, timely and efficient. Our region aligns with the two Local Health Districts – Illawarra Shoalhaven and Southern NSW.
Integrated care involves the provision of seamless, effective and efficient care that reflects the whole of a person’s health needs; from prevention through to end of life, across both physical and mental health, and in partnership with the individual, their carers and family. In practice it requires a greater focus on a person’s needs, better communication and connectivity between health care providers, and better access to general practice or an Aboriginal Medical Service (AMS). We have been working with local health providers to ensure greater service and system integration between primary and acute care.
We worked collaboratively with the Illawarra Shoalhaven and Southern NSW Local Health Districts to develop a Board approved Regional Mental Health and Suicide Prevention Plan.
The plan has nine priority areas against which key actions are identified that involve working together across the health system, with consumers and other stakeholders, to provide better integrated care for people with mental illness.
We have jointly established four GP Liaison roles with the Southern NSW LHD and one GP Liaison role with the Illawarra Shoalhaven LHD.
This is a collaborative approach to improve healthcare providers’ awareness of available services across care settings, establish and enhance communication between medical practitioners, identify system based challenges impacting on access to and quality of care provided across the region, and identify opportunities for incremental and transformational changes to improve system functionality.
HealthPathways is a free web-based portal designed to support health professionals in planning patient care through primary and secondary health care systems within the local region.
We have continued to support HealthPathways initiatives to document clinical referral pathways.
We have supported health care providers to be ready for the opt-out arrangements under the Federal Government's My Health Record expansion.

We have trialled new modes of integrated care such as the Geriatrician in the Practice program in the Illawarra Shoalhaven, and chronic pain management in Southern NSW.
