This initiative was designed to trial a quality improvement activity to improve patient outcomes and support nurses working to top of scope. COORDINARE is invited general practices to apply for funding in 2020 to implement a nurse-led clinic.
By simply submitting an online application, outlining the nurse-led clinic they would like implemented in their practice. A panel then reviewed the applications and practices who were successful received support and funding to develop and implement their clinic.
Building on the work completed for the Patient Centred Medical Home (PCMH), COORDINARE offered practices an opportunity to implement one of five Nurse-led clinic models, with the aim of delivering enhanced care for patients at risk of poor health outcomes. Practices also had the opportunity to use the Patient Activation Measure (PAM) tool at baseline and completion of the implementation of their model of care.
