Do you want to trial a quality improvement activity that will improve patient outcomes and support nurses working to top of scope? Then this is a perfect opportunity for you! COORDINARE is inviting general practices to apply for funding in 2020 to implement a nurse-led clinic.
Simply submit an online application, outlining the nurse-led clinic you would like to implement in your practice. A panel will review the applications and practices who are successful will receive support and funding to develop and implement their clinic.
Building on the work completed for the Patient Centred Medical Home (PCMH), COORDINARE is offering practices an opportunity to implement one of five nurse-led clinic models, with the aim of delivering enhanced care for patients at risk of poor health outcomes. Practices will also have the opportunity to use the Patient Activation Measure (PAM) tool at baseline and completion of the implementation of their model of care.
