Programmed Shared Medical Appointments - Australasian Society for Lifestyle Medicine (ASLM)
Shared Medical Appointments (SMAs) or ‘group consultations’ is a model of care that increases access to care, reduce costs, utilises peer support and improves consumer and provider satisfaction. COORDINARE currently funds SMA programs in the areas of weight management and smoking cessation.
Lifestyle Integrated Functional Exercise (LiFE) – Grand Pacific Health
LiFE is an evidence based program which reduces falls-risk and improves strength, balance and confidence. The program is offered in locations across the region that have a high ageing population and an identified service gap for strength and balance programs. Find out more here.
Connecting Care in the Community – Grand Pacific Health
A care coordination service improving the lives of people in the Illawarra Shoalhaven area who live with complex health needs, due to chronic conditions. This service is jointly funded by COORDINARE and Illawarra Shoalhaven Local Health District. Find out more here.

Chronic Pain Management Program – NSW Agency for Clinical Innovation (ACI) and Southern NSW Local Health District
