Through this funding, SENSW PHN seeks to support improved practice capacity and heightened quality patient care by promoting a team based care approach. Ultimately, the funding will support the integration of pharmacist as part of the care team by developing strong working relationships with both general practice and community pharmacy; and assist practices to support vulnerable patients and reduce potentially preventable hospitalisations.
A total of up to $340,000 ex GST will be dispersed through this initiative. Individual funded projects will be a minimum of $50,000 ex GST, with requirement that the pharmacist is in practice at least one full day per week. Funding is limited to 12 months, meaning funded projects must be completed on or prior to 30 June 2020. Following this, it will be up to practices to decide whether they continue their chosen service model on a self-funded basis.
