Improving communication and coordination of palliative and end of life care in Southern NSW

COORDINARE and the GP Liaison Officers are working with the Southern NSW LHD Palliative Care Service on ways to improve communication and coordination of palliative and end of life care.

Following the development of a Palliative and End of Life Model of Care, COORDINARE and the GP Liaison Officers are working with the Southern NSW LHD Palliative Care Service on ways to improve communication and coordination of palliative and end of life care for people in Southern NSW.

The Palliative Care Service will commence sending GPs their Initial Patient Assessment, which will include any follow up requests.

Please consider whether early referral to the Palliative Care service will assist with planning and managing palliative and end of life care for your patients. The SPICT tool can assist with patient assessment.

Completion of a NSW Ambulance Authorised Palliative Care Plan enables GPs to document resuscitation plans and prescribe treatment. It also assists paramedics in knowing who to communicate with, if care at home becomes difficult.

Lastly, to ensure people are provided end of life care that is consistent with their wishes, you can order up to 30 printed copies of the NSW Government Advanced Care Directive booklet for your practice by emailing MOH-OCHO@health.nsw.gov.au.
