About HealthPathways

Within South Eastern NSW there are two HealthPathways sites:

  • ACT and SNSW: this program is a cross border partnership between COORDINARE – South Eastern NSW PHN, Southern NSW Local Health District, Capital Health Network – ACT PHN and ACT Health.
  • Illawarra Shoalhaven: this program is a partnership between COORDINARE – South Eastern NSW PHN and Illawarra Shoalhaven LHD.

Why use HealthPathways

  • Clear and concise guidance

    Each pathway provides clear and concise guidance for assessing and managing a patient and includes referral information for local services. 

  • Collaborative content

    Content is developed collaboratively by general practitioners, other subject matter experts and a wide range of other health professionals.  

  • Informed by evidence

    Each pathway is evidence-informed, but also reflects local reality, and aims to preserve clinical autonomy and patient choice.
