HealthPathways is a free online tool for health care professionals for use at the point of care.

The platform provides clinical guidance on how to assess and manage health conditions, where to refer patients locally and includes resources that can be shared during appointments.

About HealthPathways

Within South Eastern NSW there are two HealthPathways sites:

ACT and SNSW

This program is a cross border partnership between COORDINARE – South Eastern NSW PHN, Southern NSW Local Health District, Capital Health Network – ACT PHN and ACT Health.

Why use HealthPathways

Each pathway provides clear and concise guidance for assessing and managing a patient with a particular symptom or condition and includes information about making referrals to services in the local health system.

Content is developed collaboratively by general practitioners, other subject matter experts and a wide range of other health professionals.

Each pathway is evidence-informed, but also reflects local reality, and aims to preserve clinical autonomy and patient choice.

To access the HealthPathways site users need to register for a personalised account and create their own username and password. To register for your personalised account, select your area below:
