Coordinating chronic health care for high risk groups

Applications close 2 December 2026
EOI-2627-01

Background

Since July 2022, COORDINARE has commissioned a regional Care Coordination program supporting people with chronic conditions and complex care needs who experience barriers to accessing health services. The program provides care navigation, case management, health coaching and brokerage support, while strengthening referral pathways and partnerships across the health and community sectors.

COORDINARE is now inviting eligible organisations to register their interest in contributing to the future delivery of chronic disease care coordination services across South Eastern NSW. This EOI will help us understand the capability, capacity and approaches available in the provider market and inform whether we proceed to a Request for Proposal.

If your organisation has the experience and capacity to deliver person-centred, coordinated care across the region, we encourage you to submit an Expression of Interest.

Learn more about this opportunity by reading the EOI Guidelines.
