Advance Care Planning and Death Literacy education program
Background
COORDINARE is seeking a provider to design, deliver and evaluate a community education program that improves understanding of Advance Care Planning (ACP) and builds confidence to have conversations about future care preferences across South Eastern NSW.
The program aims to increase community awareness of Advance Care Planning and encourage informed conversations about death, dying and future care planning.
Program details
The successful provider will design and deliver an 18‑month community education program across South Eastern NSW that builds death literacy and Advance Care Planning awareness, delivers a minimum of 24 workshops across all 12 LGAs in COORDINARE's PHN catchment, and reports on participation and outcomes in partnership with COORDINARE.
This program is designed for community members, carers, volunteers and others who want to build their knowledge and confidence in Advance Care Planning, death literacy, grief, bereavement and future care conversations.
