Question 1

Describe the most significant challenge you face in caring for patients living with or at risk of diabetes. How do you believe completing the Diabetes in Practice for Nurses program will help you address this challenge and enhance your ability to deliver high-quality, person-centred care?  

In your response, reflect on your current practice and outline how you intend to apply the knowledge and skills gained to better support General Practitioners within your practice, enhance collaborative care and improve outcomes for patients living with or at risk of diabetes - 60% (max 500 words).

 

Question 2

Please outline your interest in participating in the Diabetes in Practice for Nurses program, including your motivation for applying. Your response should also describe how you plan to complete the required eLearning modules and participate in the final interactive webinar scheduled for August 2026 - 40% (max 500 words)
