The Care Coordination Service is a free program that supports people with chronic health conditions or complex care needs to better manage their health and stay independent at home.
A dedicated Care Coordinator works with you and your GP to understand your needs, set health goals, and develop a personalised care plan.
They can help explain medical information, organise referrals and appointments, connect you with services, and coordinate your care across different providers.
