Continuing to Be Me
Continuing to be Me (C2bMe) is a FREE mental health and wellbeing program for older people living in participating residential aged care homes.
The program provides one-on-one support and group sessions with mental health professionals to help residents improve their wellbeing, build social connections, maintain a sense of purpose, and enhance their quality of life.
C2bMe also works closely with families, GPs and aged care staff to ensure residents receive personalised, holistic support.
Partnered aged care homes
C2bMe is available through participating residential aged care homes. Contact us or the aged care home directly to confirm whether the resident can access the program.
