Next Steps Suicide Prevention Aftercare - Grand Pacific Health

A psychosocial program for people aged over 16 who have presented in suicidal crisis to emergency departments or community mental health teams. Staffed by skilled peer workers who work closely with the person to help reduce their risk and take steps towards the life they want.

Palliative Aged Care Consultancy Services (PACCS)

COORDINARE has partnered with Palliative Aged Care Consultancy Services (PACCS) to help Residential Aged Care Facilities (RACFs) in South Eastern NSW provide free palliative care for their residents. A Palliative Care Clinical Nurse Consultant (CNC) facilitates Needs Rounds to RACFs on a regular basis.

The Salvation Army's Pathways Program offers adults 18 years and over, drug and alcohol education, relapse prevention and management and strategies for harm reduction. This service includes one-on-one and group support sessions, service referrals and drug and alcohol education sessions.

Psychological Therapies - Grand Pacific Health

Grand Pacific Health (GPH) provides a variety of tailored and flexible psychological therapies for people in South Eastern NSW. Sessions are free, predominately face-to-face, and can be accessed through a referral.

In partnership with new Head to Health services, COORDINARE has commissioned five new mental health hubs, to be fully operational by January 2024, delivering free, evidence-based psychological supports for vulnerable and hard to reach communities.

The Rural Outreach Mental Health Service (ROMHS) provides free clinical support from a mental health nurse to people with complex mental health issues in the Shoalhaven, Shellharbour and Kiama regions, and Southern NSW. This is a free service available via referral.
