Sentinel Practices Data Sourcing (SPDS) project: Stream 2 – Data Driven Continuous Quality Improvement

Guided by the results and evidence base generated by Stream 1 of the SPDS project, COORDINARE works with general practices to improve the overall effectiveness of routine primary care consultations through robust quality improvement principles, and by using innovative tools and several comprehensive resources.

This stream of the SPDS project focuses on supporting general practices in undertaking 'meaningful use of their practice data' through a structured and methodical approach of holistic 'clinical data auditing'.

COORDINARE provides comprehensive resources, customised training, and access to innovative tools to partaking practices in implementing catchment wide data quality improvement, enabling you to undertake regular data driven service performance and patient outcomes monitoring. SPDS QI methodology is outlined in the following videos: IntroductionStep 1, Step 2, Step 3 and Step 4

A structured and methodical process is utilised by practices to improve data quality, identify at-risk patients and regularly benchmark their performance against the regions averages, peers and exemplar practices; all aimed at improving patient and population health outcomes.

Over the past few months this stream of the SPDS project has continued to be rolled out across the entire South Eastern NSW region, with more than 150 practices now signed up. General practices interested in getting involved can contact Abhijeet Ghosh, aghosh@coordinare.org.au or your Health Coordination Consultant on 1300 069 002.
