Recent experiences of both bushfires and COVID-19 have presented significant challenges to general practice, and the way in which medical care is provided to the community.
Increasing access to telehealth and other reforms provide an opportunity for practices to modify their models of care to ensure they can continue to provide quality care to those patients most at risk of poor health outcomes.
During this time of major disruption across our communities, COORDINARE invited expressions of interest from general practice to implement a quality improvement activity focusing on the care and management of those patients who fall within the 'rising risk' cohort.
Practices were encouraged to submit an online application outlining the model of care they would like to implement in their practice. A panel reviewed the applications and practices who were successful received support and funding to develop and implement a new or enhanced service model.
