COORDINARE’s 2024/25 Annual Review now available!

COORDINARE – South Eastern NSW PHN is proud to launch our 2024/25 Annual Review, showcasing our continued progress under the Strategic Directions 2024–27: Fostering healthier communities.
This year’s review is especially meaningful as we celebrate 10 years of impact. Since our establishment, we’ve remained focused on reducing health inequities by working in partnership with communities, health care providers, and stakeholders to co-design solutions that improve access to care for those who need it most.
The review highlights key areas of work from this past year, including:
- expanding mental health support and urgent care options
- supporting general practice and the allied health workforce
- assisting people with alcohol and other drug needs
- strengthening palliative and aged care
- advancing digital health initiatives
- growing the lived experience workforce
- supporting suicide prevention teams across the region
- extending support to vulnerable communities
- deepening our commitment to Aboriginal health and cultural safety.
These achievements, and many more featured in the review, would not be possible without the dedication of our staff, the guidance of our Board and Councils, and the support of our partners and stakeholders. We also acknowledge the Australian Government for its funding through the PHN Program.
The 2024/25 year has been a defining chapter for COORDINARE’s journey, and our 10-year milestone offers a moment to reflect the progress we’ve made together. Thank you for your continued collaboration and commitment to improving health outcomes across South Eastern NSW.
