Strengthening Partnerships: COORDINARE and Southern NSW Local Health District working together for better care

On 3 December 2025, COORDINARE - South Eastern NSW PHN joined Southern NSW Local Health District (SNSWLHD) at the District Nurse Practitioner Professional Development Day in Queanbeyan. This event brought together clinicians, leaders, and partners to share insights, launch strategies, and explore opportunities for collaboration. COORDINARE was proud to present on HealthPathways and highlight how our partnership with SNSWLHD is driving systemised, accessible, and culturally safe care across South Eastern NSW.

Our connection and collaboration

COORDINARE and SNSWLHD share a commitment to improving health outcomes by working together on initiatives that make care easier to navigate and more integrated. Some of our joint achievements include:

  • Joint needs assessment – Combining data and insights to plan services that meet local needs.
  • New care models – Co-designing programs like Healthy Hearts and Minds to focus on prevention and community care.
  • Disaster preparedness – Supporting continuity of care during floods and fires through documented emergency processes.
  • Urgent Care CentresCoordinated implementation of centres in Batemans Bay and Queanbeyan to increase patient access and reduce unnecessary ED presentations.
  • Digital health – Expanding telehealth, secure messaging, and advance care planning.
  • Workforce development – Hosting joint education events, exploring Nurse Practitioner workforce models, and offering GP scholarships.

HealthPathways – Connecting care

HealthPathways is a joint program funded by SNSWLHD, Capital Health Network, ACT Health and COORDINARE. With over 680 pathways and 15,000 page views per month, it provides trusted, evidence-based guidance at the point of care. It supports clinicians with local referral options, mapped services, and resources for patients, helping ensure care is consistent and streamlined across the region.

Our partnership is not just about today—it’s about building a future where care is more connected, equitable, and responsive. Together, we will continue to focus on:

  • closing gaps in primary care access
  • strengthening culturally safe services for Aboriginal communities
  • expanding digital health solutions
  • supporting workforce sustainability through education and collaboration.

By working together, COORDINARE and SNSWLHD are shaping a health system that meets the needs of our communities - today and into the future.
