Your story, your choice this National Advance Care Planning Week

Australians are being encouraged to reflect on what matters most and make their healthcare wishes known during National Advance Care Planning Week (16–22 March 2026).
With this year’s theme, Your story, your choice, the campaign highlights the importance of documenting personal values and treatment preferences so they can guide care if someone becomes seriously unwell and unable to communicate.
For Paul and Heather, advance care planning provided clarity during a sudden and frightening health emergency. When Paul experienced an unexpected cardiac arrest and was unconscious for 18 hours, Heather was asked whether he had any advance care planning documents in place.
“My immediate reaction was relief and gratitude,” Heather said. “Knowing his wishes meant I didn’t have to make those decisions alone. It gave me confidence that I was honouring what he would have wanted.”
Paul said completing an advance care directive years earlier gave him peace of mind, never imagining how soon it might be needed.
“It became a document that speaks in the silence,” he said.
Prue Buist, CEO of COORDINARE - South Eastern NSW PHN, said advance care planning is important for people of all ages — not just older Australians.
“Research shows many Australians will not be able to make their own medical decisions at end-of-life, yet only a small proportion have documented their wishes,” Ms Buist said.
“Advance care planning ensures your values guide your care and provides clarity and comfort for families during what can be incredibly stressful times.”
Ms Buist encouraged people to consider who would speak for them if they were unable to communicate, and whether that person understands their preferences.
National Advance Care Planning Week is an opportunity to start the conversation, appoint a substitute decision-maker, and document your wishes.
