Join the LinkMyCare Program in South Eastern NSW
COORDINARE – South Eastern NSW Primary Health Network (SENSW PHN) is offering general practices support to improve outcomes for their patients with chronic airways disease.
This opportunity provides funding for practices which includes:

purchase of spirometry equipment
-
respiratory physician led spirometry assessments which are free for patients
-
staff training in spirometry by a respiratory physician
-
access to pulmonary rehabilitation which is free for patients
-
practice support to identify eligible patients
-
funding for nursing time to support eligible patients and deliver structured COPD care plan
This funding opportunity includes $2000 to facilitate set up and then up to $395 per year per patient enrolled in the program. This funding does not impact regular MBS item billing.
Chronic airways disease remains one of the leading causes of potentially preventable hospitalisations across our region. LinkMyCare is part of Collaborative Commissioning, a NSW Health initiative which is working to support patients in managing their condition, and to shift care from hospital settings into the community.
Participating practices will have enhanced collaboration with LHD services when patients are discharged after hospital admissions to ensure continuity of care.
