Background

LinkMyCare – COPD is a care pathway designed by local clinicians and consumers to provide enhanced care to people with chronic obstructive pulmonary disease (COPD) 

The service will be available across 16 general practices throughout South Eastern NSW for up to three years. The program will provide resources for those practice teams to support patients to manage their COPD in the community and access the services they need without travelling far from home or paying any out-of-pocket costs. 

The pathway will also support patients to move seamlessly between hospital and their general practice, reducing the likelihood of a re-presentation to hospital and improving the information flows between a patient’s GP and the care team within the hospital. 

This EOI seeks to commission allied health providers to deliver pulmonary rehabilitation in:

  • Northern Illawarra
  • Queanbeyan

COORDINARE - South Eastern NSW PHN is looking for proposals from allied health providers, experienced in delivering rehabilitation services and an interest in delivering pulmonary rehabilitation.   

The successful provider should be familiar with the SENSW health service system including understanding current services available and current challenges in the region. Providers should be available to deliver on the outputs stated in this EOI across a two-year period.
