Background
What is the Healthy Hearts Program?
The Healthy Hearts Program (HHP) supports small general practices and Aboriginal Community Controlled Health Organisations (ACCHOs) to strengthen prevention, early detection and management of cardiovascular disease (CVD) and CVD risk in primary care.
Participating practices host a HHP staff member - either a Registered Nurse or suitably qualified allied health professional - one day per fortnight. The HHP staff member is employed by COORDINARE’s commissioned provider (Silverchain Ltd) and works alongside the practice team to support patients with CVD or those at risk of developing CVD.
The program focuses on:

identifying patients who would benefit from CVD risk assessment or enhanced management
-
supporting shared care planning and multidisciplinary care
-
addressing modifiable risk factors such as medication adherence, lifestyle and referral to allied health.
What does participation involve for general practices/ACCHOs?
Successful practices will:
-
identify and refer eligible patients into the program
-
provide a suitable consultation space one day per fortnight
-
support practice workflows to host Healthy Hearts clinics
-
participate in multidisciplinary care alongside the HHP staff member.
Successful applicants are not expected to employ additional staff as part of this program but to host a HHP staff member (employed by Silverchain Ltd) clinic once per fortnight.
COORDINARE currently supports eight small practices through the HHP and is seeking up to ten additional practices and/or ACCHOs to participate from July 2026.
RFP details
This opportunity is open to ACCHOs or independently owned and operated general practices that have three or fewer GP fulltime equivalents. Please refer to the RFP guidelines for full eligibility requirements.
Successful applicants will receive:

a one-off setup grant of $15,000 (ex GST), and
-
participation in the HHP, including a visiting HHP staff member one day per fortnight.
