The C2bMe@home program supports the emotional and mental wellbeing of older people living in South Eastern NSW who are experiencing mild mental health concerns.
The program helps individuals to continue living with purpose and dignity in the community by providing up to 6 hours of support sessions. Support is delivered in the home of the older person or via telehealth.
Support is delivered by a mental health professional who provides short-term strategies that are evidence-based and person-centred.
