Healthy Hearts
Healthy Hearts is a FREE program designed to support people to improve their heart health and overall wellbeing through proactive, community-based care.
Working closely with your GP, experienced Silverchain nurses provide personalised support to help you better understand your cardiovascular risk and take practical steps to improve your health.
How the program supports you:
- A comprehensive heart health assessment using the Heart Foundation’s Heart Health Check Toolkit
- A personalised action plan tailored to your goals and risk factors
- Ongoing health coaching to build confidence and support self management
- Connections to local services, including dietitians, exercise physiologists, and community programs such as walking groups
- Access to reliable resources and digital tools to support your health between visits
- Coordination with your GP and other providers to ensure joined up, high quality care.
