Integrated Team Care - Illawarra Aboriginal Medical Service
The Integrated Team Care program supports eligible Aboriginal and Torres Strait Islander people with chronic health conditions to access healthcare and support services. Illawarra Aboriginal Medical Service provides culturally safe care coordination to help clients manage chronic disease, access treatment and stay connected with their healthcare team.

Who can access this service
This service is available to people of all ages who identify as Aboriginal and/or Torres Strait Islander and
-
are living with a chronic health condition that has been present for six months or more;
-
have a current GP Chronic Condition Management Plan (GPCCMP); and
-
have been referred by their GP.
To access this service, you'll need a referral from your GP along with a current GP Chronic Condition Management Plan (GPCCMP).
