Establishment of a Care Coordination Program

COORDINARE - South Eastern NSW Primary Health Network invites proposals from eligible organisations to establish, deliver and manage a Care Coordination Program to support primary care and contribute to better health outcomes for people living with chronic and complex conditions in the South Eastern NSW region.

Applicants are invited to propose models that articulate multi-disciplinary, person centred and coordinated care, including health coaching, care navigation and case management.

The Care Coordination Program will operate regionally working alongside engaged general practice, in the community by supporting practice attendance, delivering home visits and telehealth support.
