St Vincent’s Hospital Sydney Chronic Pain Service
The St Vincent’s Hospital Sydney (SVHS) Chronic Pain Service provides an Outreach Chronic Pain Management Education and Service for the Southern NSW catchment area.
The outreach service provides telehealth, multidisciplinary support and outreach site visits for clinical service delivery and clinician training and education.
This service is a collaboration between the SVHS Pain Team, COORDINARE, and the SNSW LHD.

Chronic Care Coordination program
This program provides a dedicated person to help community members understand their health, coordinate appointments and services, and stay as healthy and independent as possible while living at home.

Gateway Life are offering free access to an 8-week gym-based program combining exercise and education for people aged 45 and over who are at risk of developing chronic conditions. Participants will attend weekly group sessions, alternating between physical activity and education.
