Shared Medical Appointments (SMAs) or ‘group consultations’ is a model of care that increases access to care, reduce costs, utilises peer support, and improves consumer and provider satisfaction. SMAs target a particular chronic condition and aim to improve health outcomes.

Building on the success of the Integrated Team Care services, COORDINARE has partnered with the Silverchain Group to expand access to the care coordination services across South Eastern NSW. The Chronic Care Coordination program is available for people with a chronic health condition and complex health needs.

Snowy Monaro Palliative Care Volunteering provides a FREE volunteer support service to people living with a life-limiting illness as well as their families and carers. Their specially trained volunteers can provide support by phone or video calls or by visiting people at their home.

South Coast Medical Service Aboriginal Corporation (SCMSAC) delivers the 'Strong Foundations' school-based mental health literacy and help-seeking program for Aboriginal and Torres Strait Islander children in years 3–6. Social and emotional wellbeing service for family members requiring additional support.

Strong Yawa provides an after hours support line for Aboriginal women and children, delivered by Waminda. Available after 5pm, all calls are confidential and will be treated with respect.

Suicide Call Back Service

Suicide Call Back Service offers free professional 24/7 telephone counselling support to people at risk of suicide, concerned about someone at risk, bereaved by suicide and people experiencing emotional or mental health issues.

Telecare for Kids

Provided by Royal Far West, Telecare for Kids is a program that connects children across regional and remote Australia to allied health services via video-link, without needing to leave their local community. Telecare for Kids is available to children aged 0–15 and their families living in South Eastern NSW.

The Disability Trust - Commonwealth Psychosocial Support Program

Free non-clinical support for people 16 years and over with a severe and complex mental health condition. Also provides support to apply for NDIS if primarily 'Psychosocial Disability' and Service Navigation. Available via telehealth and across multiple locations.
