Service Navigation

COORDINARE's Service Navigation for Psychosocial Supports has been created to help support people to identify and locate the most suitable mental health services for their needs. It can be accessed by consumers, their families, and carers, as well as GPs, allied health, and community practitioners.

Service Navigation enables early and easy access to health services by promoting available psychosocial programs in our region. It helps people to navigate the range of Community supports that are available and discover which best meets their needs.

Service Navigation can be accessed by anyone wanting information to assist people who:

  • live in the COORDINARE South Eastern NSW PHN region (see map)
  • have a severe mental illness and associated psychosocial functional impairment
  • need information or support to access mental health services or NDIS supports.
