Services we fund
Working in partnership with Aboriginal Community Controlled Health Organisations (ACCHOs) and Aboriginal people is a key priority for us to address unacceptable health inequalities and improve health and wellbeing. We commission targeted services to Aboriginal people needing care coordination, mental health and suicide prevention, and alcohol and other drugs support.

South Eastern NSW has a high percentage of adults who consume alcohol at levels posing a long-term risk to health. We commission a number of services to support people with alcohol and other drug-related issues, prevent harm, and build capability in primary health care and our communities.

The prevention and management of chronic conditions is an important area of work for us. We commission activities that reduce lifestyle risk factors, enhance the management of chronic conditions, and increase the efficiency and effectiveness of medical services for patients at risk of poor outcomes.

We are committed to improving health experiences and outcomes for older people, and those receiving palliative and end of life care. We commission services for people living with life-limiting illnesses, and which support end-of-life planning.

There are high rates of mental and behavioural disorders, and psychological distress in our region, especially amongst Aboriginal people and people living in rural areas. We work with our communities, consumers and partner organisations to deliver targeted and integrated mental health care and support across the region.
We commission partners to deliver health-promoting and preventative mental health services, support GP capability, and provide care for people living with mental health issues.
